Coordinate and manage care for Aetna members in the North/West side of Chicago, IL. Utilize case management skills and experience to ensure efficient and effective delivery of healthcare services. Conduct assessments and develop care plans for members in accordance with Aetna guidelines. Communicate and collaborate with healthcare providers, vendors, and other stakeholders to ensure the best possible care for members. Maintain accurate and up-to-date records of member information and progress. Utilize critical thinking and problem-solving skills to identify and address any barriers to care. Adhere to all regulatory and compliance requirements. Participate in team meetings and contribute insights and ideas for improving processes and services. Serve as a resource for members, answering questions and providing support as needed. Continuously evaluate and monitor the effectiveness of care plans and make adjustments as necessary. Stay informed about industry advancements and changes, and incorporate new knowledge into daily practices. Demonstrate empathy, compassion, and understanding towards members to ensure a positive and supportive experience. Maintain a high level of organization and attention to detail in all tasks and responsibilities. Prioritize and manage a caseload of members efficiently and effectively.
